You will need these items when registering your child.  The first 5 items are required by the State of Georgia.


1. Birth Certificate:

2. Georgia Certificate of Immunization (Form 3231-revised 3/07): If you are
coming from out of state, this record needs to be taken to a Georgia physician or   
the Dawson County Health Department and transferred to the revised  Georgia
Form.

3. Eye, Ear & Dental Certificate (GA Form 3300):  from you doctor or the County   
Health Department.

4. Social Security Card:

5. Proof of Residency: deed, lease, utility bill, etc. at the
address you reside.

6. Name, Address, Phone and Fax number for previous school.
